Welcome! Registration is an exciting time for us. It means that a brand new school year is right around the corner and we are eager to welcome our students, parents, and teachers back to Locust Grove.

Unfortunately registration is a somewhat tedious process and involves some work on our parents’ part, but we hope our website will make this a little easier and quicker this year. Please read the following instructions very carefully. The safety of your child is of the utmost importance to us and it is critical that we have accurate and current information. Some items below include links to forms for you to fill out and print using the free Adobe Reader software. Most computers come with this software installed, but if you do not have it you can download it here.

Registration is on Tuesday, July 27 and Thursday, July 29 from 8:00 am to 6:00 pm.

The following are required for each student:

• Enrollment Card – Click to complete and print form (if you are enrolling multiple students, you can print, and then edit just the information different for each child.)
• Health/Photograph/Hearing/Vision/OCSNET AUP – Click to complete and print form – this form contains four separate forms including a health/custody form, 4068-F Permission to photograph/record, Hearing and Vision Screening Permission, and the OCSNET Student Acceptable Use Policy User Agreement Form (link to OCBE Acceptable Use Policy 9064)
• Proof of Residence – Please bring a utility bill verifying proof of residence.
• Health Insurance Card – (will be copied and returned)
• $15.00 Basic Student Fee
• $40.00 for School Nurse – Click here for more information

The following are required of students new to Locust Grove:

• All of the above items
• New Student Checklist 9005.022-F – Click to complete and print form
• Kentucky Immunization Form (from your physician)
• Kentucky Physical Form (from your physician)
• Social Security Card (we will copy and return)
• Original Birth Certificate (we will copy and return)

The following are required of Kindergarten students only:

• All of the above items
• Transportation of Kindergarten Students Form – Click to complete and print form
• $350 for Kindergarten Tuition
• Get Acquainted Program (GAP) – Click to complete and print form

Criminal Records Check

The criminal records check is required every year for any parent or guardian that is interested in volunteering in the school, attending field trips, and/or eating lunch with students at school. Please fill out a separate form for each adult. A short confidentiality training is also provided early in the year by our counselor for any volunteers or lunch room visitors.
